Using peek fingertight fittings on the inlet to the column is the problem here. As you increase the flow, the pressure pops the tubing out of this fitting.
Better to use stainless steel nuts and ferrules on the high pressure end of the column, like you have on the outlet.
Even better, you could use the InfinityLab Quick connect or Quick turn fittings: https://www.agilent.com/cs/library/flyers/public/flyer-quick-connect-fittings-infinitylab-5991-5164EN_agilent.pdf making sure that you have the correct tubing with the "long socket" .
