There is something wrong with the tray or the sampler itself.
If you have another vial sampler you can swap the vial trays.
Thank you for the Reply. We resolved this issue by changing over to a new set of vial trays and it fixed the issue. I also found out later that over time, the reflective contact surfaces start to deteriorate and hence cause issues. An online support engineer actually suggested to use a correction pen to white out the contact surfaces and they should be good to go. I have also successfully tried this.
